P2Y12 receptor antagonists

Mechanism of Action

  • P2Y12 receptor antagonists block P2Y12 receptor which is an ADP receptor → inhibition of platelet activation & aggregation

Examples

  • Clopidogrel (Plavix®)
  • Ticagrelor (Brilinta®)
  • Ticlopidine
  • Prasugrel

Clopidogrel (Plavix®)

  • Drug interactions
    • Certain proton pump inhibitors may reduce effect of clopidogrel
  • Dosage
    • ACS: T Clopidogrel 300 mg stat, then 75 mg OD

Ticagrelor (Brilinta®)

  • Clinical use: STEMI awaiting for primary PCI
  • Dosage: T Ticagrelor 180 mg stat, then 90 mg BD
  • Contraindications
    • High bleeding risk; H/O intracranial haemorrhage
    • Severe hepatic dysfunction
  • Cautions
    • Asthma, COPD

Prasugel

  • Contraindications
    • High bleeding risk
    • History of stroke
    • Hypersensitivity to prasugel
