• Used to assess bleeding risk in a patient with atrial fibrillation started with anticoagulant
Components
- Hypertension (SBP >160 mmHg) — 1 point
- Abnormal renal and or liver function — 1 or 2 points (1 point for each abnormality)
- Renal: ESRF, requiring dialysis /renal transplant
- Liver: cirrhosis / serum bilirubin 2×normal / AST or ALT 3×normal
- Stroke — 1 point
- Bleeding — 1 point
- Requiring hospitalization / blood transfusion
- Hb reduced >2 g/dL
- Haemorrhagic stroke
- Labile INR (<60% in therapeutic range) — 1 point
- Elderly, i.e. >65 years old — 1 point
- Drugs and/or alcohol excess — 1 or 2 points (1 point for each — drug / alcohol excess)
Interpretation
• Score ≥3 suggests high risk / caution in using warfarin
