CHA2DS2VASc Score

If patient has atrial fibrillation, this CHA2DS2VASc Score should be done to assess the 1-year risk of thromboembolic event in non-anticoagulated patient with non-valvular atrial fibrillation

Components

Congestive heart failure history – 1 point
Hypertension – 1 point
Age >75 years old – 2 points
Diabetes mellitus – 1 point
Stroke / TIA previously – 2 points
Vascular disease (H/O MI / peripheral artery disease)- 1 point
Age 65 – 74 years old – 1 point
Sex category – Female – 1 point

Interpretation

Score 0 (low risk): may not require anticoagulant unless valvular atrial fibrillation
Score 1 (low-moderate risk): should consider anticoagulant / antiplatelet
Score 2 or more (moderate-high risk): anticoagulant is recommended

Notes

  • Transthoracic echocardiogram should be done TRO valvular heart disease
